Dr. Leung's notebook
Sunday, March 3, 2024
Based on the "Beijing-Changzhi Medical Cooperation" program, I was invited by the Department of Neurosurgery of Heiji Hospital in Chang zhi, Shanxi Province. I completed two brain surgeries over the weekend, and at 8:00 p.m., I was ready to go back to Beijing on the CA1120 flight from Changzhi-Beijing. Soon after the plane took off, still in the rapid ascent period, suddenly the neighboring passengers gently but urgently pushed me, I turned my head to look, startled: I saw her hand on the chest, gray face, eyes closed, frowning, expression of pain, rapid breathing. She whispered intermittently that she panicked, shortness of breath, chest tightness and discomfort, let me ask the stewardess to come over, and she said she wanted to get off the plane immediately.
This woman is 60 years old and above, because I do not know her usual health condition, my initial judgment is either the rapid ascent period of the aircraft, the feeling of overweight pressure and emotional tension caused by the vagus nerve reaction, or the terrible angina attack. At that moment in time, I can only think of the bad first. If it was angina, it could be cardiac arrest at any time, requiring a race against time to save the patient. My brain was conditioned to calculate what medication should be given and what was available at the moment, and if in the unlikely event of respiratory and cardiac arrest, where should external cardiac compression and artificial respiration be performed, and I even thought about how to mobilize the emergency evacuation of first-class passengers to make room for first aid if needed. At that moment, all passengers and flight attendants were fastening their seat belts and staying in place to cope with the physical discomfort caused by the vibration of the airplane during the ascent period. The situation was urgent. In the noise of the aircraft, I and the stewardess, who was fixed to the seat a few meters away, spoke with signs and told a passenger is sick, urgent need for help. But at 10,000 meters, where would the flight attendant find a doctor? Even if she came over immediately, she is not a professional doctor, what can she do? Even if there are emergency medicines on the airplane, she may not dare to use them or know how to use them. I am a doctor, who else can I be? As a doctor, I travel on high-speed trains and airplanes all the time, and occasionally I would hear announcements to look for a doctor, and I would come across passengers in need of medical help, and I have come to my aid on many occasions. Although the vast majority of them are safe and do not require any special treatment, some of them do require medication. So I then deliberately prepared some common medicines, such as fast-acting heart pills, prednisone, painkillers, anti-allergy drugs, etc., and carried them with me in case of emergency.
My seat near the aisle, I "illegally" unbuckled the seat belt, quickly stood up, "illegally" opened the luggage compartment, took out the backpack, quickly found out the fast-acting heart pills, poured out a few small tablets from the zucchini, and let the woman open her mouth to take the medicine with the help of me and the passengers by the window! I touched her pulse. Not bad! The heart rate was not too fast, about 80-90 beats per minute. The pulse was quite strong, at least there was no obvious arrhythmia and pulse count. I held her hand and whispered in her ear to reassure her that it was nothing serious, don't be nervous, don't talk, breathe normally. I also told her that I was the doctor and that I would protect her throughout the entire journey. Passengers in the front and back looked at our area with a slight look of panic in their eyes. Although I have been a brain surgeon for more than 20 years, I have seen a lot of life-threatening emergencies, but at this time I was also very nervous, after all, 10,000 meters in the air, single-handedly, without supervision, without laboratory tests, limited medicines, narrow space, and in full view of the public, who knows what will happen to the woman next me? But I knew that I couldn't show my nervousness even if I was nervous. The doctor should be the most calm person on the scene. I held her hand, judging her pulse and conveying love, confidence and encouragement to her, hoping to alleviate the fear, loneliness and helplessness she felt when the disease struck. Time passed in a tense atmosphere, I did not know whether it was the role of drugs, or psychological comfort, or the aircraft rose to the intended altitude, there was no longer a feeling of overweight pressure, perhaps a combination of roles, about 15 minutes later, her situation gradually stabilized, her expression was not as painful as the original, the eyebrows were gradually stretching, and the eyes were slowly opening. I gently asked her how she was feeling, and she said she was much better than before.
During this time, the stewardess came over and asked about her feeling, and when she learned that I was a doctor, her eyes lit up and she was very happy and thanked me sincerely. The man sitting near the window helped her unscrew the water cap, his eyes full of concern. The eyes of the passengers around us also changed from horror to calmness, with strong praise. Her condition was in remission, and I was relieved and in a good mood. The plane east across the "Taihang eight defile" of the "Busy mouth defile", to the airspace over Handan. "The sky is ten thousand miles long, looking down at the mountains and rivers". The battle of Changping, Lian Po along this throat, from Handan to Shangdang, resistance to the Qin, and then Zhao Kuo replaced Lian Po, Zhao defeated, more than 400,000 Zhao troops were killed by the Bai Qi pit ...... Surprisingly, in this congealing thick history of Taihang Mountains above, I met with medical assistance unexpectedly. To cure diseases and to lead troops in battle, to separate lines without separation, we need to be calm, accurate judgment and decisive action.
The woman's condition became more and more stable, her face became more and more beautiful, and her eyes gradually gained luster. I also dared to communicate with her. I asked her how old she was and whether she had any heart problems. She said that she was 57 years old, and that there were occasional times when her heart area was uncomfortable, and she had also had an EKG done, but there didn't seem to be any major problems. So I instructed her that she still needed to go to the cardiology department to have a good checkup, and then asked her to rest quietly.
In the quiet night sky, the airplane arrived over Hengshui, east of Shijiazhuang. From the top of the Taihang Mountains in Shangdang to Beijing at the foot of the Yanshan Mountains, it is only an hour's flight. The plane passed Langfang, and the already clearly visible light on Beijing ground showed the plane was about to land. The flight attendant once again asked with concern how the woman was feeling, and asked me to assess whether it was necessary to contact the airport ambulance. I said that everything was fine, so I would take her out of the airport. I asked her where she was from, what the purpose of her trip is and whether there is someone to pick her up at the airport? She told me that she was from Changzhi, her daughter was a civil servant working in Changzhi, and her son-in-law was studying for a doctorate in Beijing. She came to Beijing to see a gastrointestinal disease, and her son-in-law would come to the airport to pick her up later. During the chat, I learned that her son-in-law's doctoral degree was in medical testing. I was very excited, then my son-in-law and I are not peers? I couldn't help but ask which hospital her son-in-law was studying for his doctorate, and she said Xuanwu Hospital of Capital Medical University. Wow, it's really getting closer and closer, I said I am the neurosurgeon of Xuanwu Hospital ......
The plane landed at T3 terminal of the Capital International Airport, and when I got off the plane, several flight attendants greeted to us and expressed their deep gratitude to me. In the airport ferry, I also learned that her son-in-law is from Jiexiu, Shanxi Province. I was again amazed at the fate between people, because I also come from the cold food Qingming hometown of Shanxi Jiexiu ...... I accompanied her to the airport exit, met her son-in-law, a handsome boy who looked at me with a confused face. I smiled and introduced myself: fellow townsmen, peers, and we are still working in the same hospital. And I told the ins and outs of the airplane, the young man thanked me again and again. Kindly talk to know, the young man and I come from the same high school - jiexiu city first high school. I was in Class 108 of 1988 while he was in Class 269 of 2005. His name is Wang Xiaoling and he is my junior male shoolmate. Go round and round, the woman I helped is actually his mother-in-law. It is estimated that the best screenwriter could not come up with such a "coincidence book".
This morning, Xiaoling came to my office to visit me, his senior male schoolmate. We talked a lot and took a photo together.
Thank God, the "Battle of Shang Dang" at 10,000 meters above Taihang Mountain was no danger. Looking back, the woman probably was the vagus nerve reaction caused by the rise of the plane. The symptoms can be slowly relieved on their own without any drugs, but we can not be hindsight, at that moment, in case it really was angina attack? In addition, the Chuanqiong and ice tablets in the quick-acting heart pills were helpful in relieving my elder sister's symptoms, and the emergency medication gave her a great sense of security at the critical moment, so I am still glad to have the medication with me and will continue to carry it with me, always ready to help those in need. My teacher, Professor Ling Feng, founded the China Volunteer Doctors Organization in 2017, which provides free help to people in need of medical assistance through "volunteer medical care, poverty alleviation, disaster relief and foreign aid" and other ways. On the National Day in 2021, Shanxi Fen River was flooded, I was entrusted by Prof. Ling Feng to send doctors and medicines to the affected people in Jiexiu, and organized a volunteer medical mission for doctors of Jiexiu nationality.
So far, China's volunteer doctors organization has absorbed more than 6,000 doctors across the country, and I am also one of them. We have a "bright card action": as long as the patient has to call for help, the doctor should be answered. No matter where you are, China's volunteer doctors must have such an attitude. Individuals helping individuals may be insignificant, but "Do not fall to do good even if it is small". Many hands make light work. Together we ignite a flame; scattered we are all stars. The world is so small, we have to work hard to make it full of warmth and love.
